They simply SCAN the FRESHMEN QR CODE, Make an account, put head shot on and Enter contacts. Notes: The Avondale teen health center is open in the summer. If any student is in need of a PCP or physical, regardless of if they have insurance or not, please have them call us at (248)537-6235 or stop in Monday thru Thursday, from 7am to 3pm. We do not charge the students for physicals or any other visit. We do require a consent form filled out by a parent or guardian prior to their visit. The form is attached.
